Attention Sweepstakes Winners!

In order to collect your winnings, the promoter of the sweepstakes you won requires you to provide them with a signed publicity release to publish your name and likeness for promotional purposes. This is a standard requirement, and is incorporated in the rules of the sweepstakes.

If you won anything valued in excess of $600, federal law requires the promoter to have you complete a W-9 form in order to send you a 1099 form referencing your winnings as taxable income (the government always wants its cut). You can download a W-9 form here.
